What You Need to Know About Transition and Transfer

If you are a young person being treated at a children’s (paediatric) hospital, you will eventually need to be transferred to an adult kidney care service.

Moving from children’s care to adult care can feel like a big change. It can bring up a mix of feelings.

Some people feel ready. Some feel unsure. Most feel a bit of both. It is important to know that you do not have to figure it all out at once.

This is a process, and your healthcare team is there to guide you through it. 

The aim of this module is to provide you with the information you need to understand the process used when you move from paediatric to adult care with the aim that you will: 

  • Understand what “transition” to adult care actually means 
  • Know what to expect at your first adult clinic appointment 
  • Feel more confident speaking up and asking questions about your health 
  • Feel prepared about the move to adult service
